AccentCare is seeking a dedicated Weekend Physical Therapist to provide direct patient care in a home health setting as part of our comprehensive team. This role involves assessing patient needs, developing individual therapy plans, and administering physical therapy techniques to assist patients with injury recovery, pain reduction, and improvement of mobility and overall health. The ideal candidate will be passionate about home health care and committed to enhancing patient quality of life through exceptional therapeutic services. Conduct patient evaluations to determine physical condition, limitations, and personal goals. Develop tailored treatment plans based on patient evaluations and doctor recommendations. Administer physical therapy techniques that comply with prescribed methods. Educate patients and families on treatments, therapy exercises, and preventive techniques. Document patient care and therapy outcomes for continuous evaluation and treatment adjustment. Coordinate with other healthcare providers to ensure optimal patient care. Maintain ethical standards and continue professional development to ensure quality of care. Strong understanding of physical therapy modalities and techniques. Excellent patient-handling skills. Ability to perform accurate patient evaluations and handle medical documentation. Strong interpersonal and communication skills. Proficiency in using therapy equipment and ensuring safety protocols. Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapist license in the state of practice. CPR certified.
