Job Description:

AccentCare is seeking a dedicated and compassionate Physical Therapist to join our team, providing home health services over weekends. In this role, you will be primarily responsible for delivering physical therapy services to our homebound patients, focusing on enhancing their mobility, reducing pain, and fostering overall independence in their daily activities. By working closely with patients and their families, you will craft personalized care plans that promote optimal health and functionality in the familiar comfort of their own homes. Conduct patient assessments to evaluate their physical condition and limitations. Develop and implement tailored treatment plans that align with patient-specific health goals. Utilize various therapy techniques and equipment to alleviate pain and improve physical functions. Educate patients and family members on the therapy process and home exercise programs to ensure continued progression. Regularly document and report on patient progress and adjust treatment plans as necessary. Comply with all legal and ethical guidelines pertaining to home health care. Proficiency in various physical therapy techniques and tools Strong interpersonal and communication skills Ability to work independently and manage a flexible schedule Keen observation skills and attention to detail Capacity to handle sensitive situations with compassion and professionalism Medical Insurance Dental Insurance Vision Insurance 401k Current state license as a Physical Therapist Valid driver’s license and reliable transportation CPR certification preferred
